He will ask you how you learned about CarePatrol, if you have contacted any online agencies and assure you that CarePatrol’s service is always at NO COST to families because we are compensated by our national network of licensed providers.
He will serve as your professional advisor and guide to assist you in making an educated and safe care decision for your loved one. If you’ve never experienced having to make a long-term care decision for a loved one, it can be stressful and overwhelming. It’s easy to make mistakes, due to know fault of your own, because there is so much to know about long-term care options in our area.
Following the initial phone call, he will schedule a “care discovery” meeting with you preferably at your home where you are most comfortable. If you prefer, he will come to you at the hospital, rehab facility or a public place. The goal of this meeting is to help Mark get up-to-speed on your loved one’s care needs, budget and living preferences. He will also do his best to address your questions/concerns and refer to local professionals if necessary.
With the information provided (treated with confidentiality), he will schedule, pick you up and accompany you on tours to 2-3 safe options that meet your loved one’s level of care, budget and living preferences.
Following the tours, he will discuss the advantages and disadvantages of each care provider to help you make the safest placement decision. Once a selection has been made, he will coordinate the medical paperwork required by the Arizona Department of Health Services between the primary care physician and provider.
Mark will be with you from start to finish…from the first phone call to follow-up calls after placement.
